Proven Solutions for Severe Pain Caused by Temporomandibular Joint (TMJ) Dysfunction Syndrome

Treatment Overview – Phase I

Step 1: Dr. Goldman will ask you to complete a detailed questionnaire about your health history, symptoms, past treatments, etc.

Step 2: Dr. Goldman will take high quality panoral x-ray using the state-of-the-art CommCat Tomographic system

Step 3: Dr. Goldman will meet with you one-on-one in a non-hurried atmosphere and do a very thorough comprehensive history so you can describe your situation, symptoms as well as your prior treatments, if any.

Step 4: Dr. Goldman will perform a thorough physical exam to assess jaw movement and jaw sounds, the way in which the teeth fit together, and any spasms in the head and neck muscles as well as dental damage that was done by this problem.

Step 5: Dr. Goldman will discuss his findings and present his treatment recommendations.

Step 6: Once you agree to the treatment plan, Dr. Goldman will perform additional Tomographic x-rays and fit you with a custom, precision-designed dental orthotic device designed for your unique problem.

Step 7: On a weekly basis, Dr. Goldman will relax the jaw muscles to enable the lower jaw to assume a more relaxed position. Then, and only then does he adjust the orthotic device to accommodate the new relaxed position. Weekly visits are vital because as the jaw starts relaxing and moving (although the movement may seem miniscule) between visits, the teeth will no longer be hitting the orthotic device correctly and protective muscle spasms will return – in effect halting treatment. The purpose of this phase is to allow the lower jaw to relax and find its own healthy position, free from muscle spasms and reflexes.Patients usually start responding to treatment within 4 visits. Dr. Goldman continues this treatment until:

  • The patient’s pain is alleviated
  • The jaw muscles are calm
  • The lower jaw is relaxed to its normal physiologic position, free from the influence of muscle spasms and protective reflexes, and has assumed the normal position determined by the patient's unique anatomy of the temporomandibular joint, free of muscle spasm and protective reflexes

Treatment Overview -- Phase II

Once Phase I of treatment is complete, Dr. Goldman will assess what can be done to permanently resolve your problem by achieving proper tooth-temporomandibular joint gearing, which can only be determined at the close of phase I. If necessary, Dr. Goldman will work with your dentist or another specialist to achieve this goal.
